Subject: [roots] 9 year recall Case Surgery with 5 canals and fused DB-P root

Hereís the surgery on this tooth today.  I injected a carpule of Lido + 1:50,000
epi into the lesion site before flapping and the hemostasis was excellent throughout 
the procedure.  Very uneventful, routine surgery.

I submitted the lesion for biopsy but it looked like your garden variety granulation 
tissue. It came out in two large pieces.  Iím sure a fragment of it is still attached 
to the sinus membrane because I didnít try very hard to remove it all and devitalize 
the circulation of adjacent teeth.  A preoperative CBCT was very helpful and needed 
for this case.   Methylene Blue dye demonstrated grooves between the
DB1 and DB2 and the MB1 and MB2.  I troughed the DB2 to P anyway, when in doubt, 
trough it out - Terry
