Confluent canals are notorious for instrument separation.... 
this was a tough case, the mesials were LONG,
I fractured a file, got it out, and finished the case.
Yesterday my hygienist took a 1 year follow-up rad. I don't 
like the spots on the phosphor plate. Yes, I am impossible to 
work with...:-)), there is always something.....:-)) - Marga
