Maxillary second molar : deep division

From: Siju Jacob To: ROOTS Sent: Tuesday, October 07, 2008 3:27 PM Subject: [roots] deep division Here is a case where the scope really helped. Maxillary second molar. After completion of initial access, only two canals could be seen. Troughing and widening of the buccal canal using ultrasonics revealed division of the single buccal canal into two. To make my life a bit more intresting, the canals were long and curved and the patient kept headbanging to the song on the radio. Pictures are self- explanatory. - Siju

Hi Siju.....Fantastic case. Do you normally trough the buccal canal in this fashion or is it in this case cos' you didn't find the Distobuccal ? Also would you consider this as the Distobuccal or as the Mb2? - Sachin Hi Sachin, If I find only two canals in a maxillary second molar, I always look for the third canal. many a times, both buccal canals orignate from one orifice. In this case, the first canal I found was the disto-buccal, the second was the mesio-buccal. - Siju that's a beauty--were the buccal canals confluent? - Kendel Thanks Kendel, Yes; the buccal canals were confluent. - Siju Considering that you are using a digital dental x-ray system which system are you using? Which X-ray machine are you using your X ray pictures of upper second Molars seem to be perfect. Excellent finish. See that over hanging "prong" from the neighboring molar? May be you are going to round of that in the subsequent sitting.- Sanjay Jamdade Thanks Sanjay. I'm using a suni digital radiograph. I'm not too happy with it. I don't always get such clear radiographs. I plan to hopefully move on to a kodak 6100 soon. The x-ray machine is satelec. - Siju Good one Siju . Nice BU also.Keep raising the bar! - Rajiv Patel Thanks Rajeev!- Siju Hi Siju, Very nice case - Noemí
