Subject: [roots] Funky distal root......

5 canals were identified, debrided, disinfected and obturation performed
using thermo-softened gutta-percha and resin sealer. The floor of the chamber
and orifice interface of dentin and gutta-percha were bonded with a dentinal
adhesive and flowable composite to prevent recontamination. You will need
to refresh the axial dentin and bond the chamber prior to fabricating the
post/core complex. An oral conscious sedation technique was used for this
procedure. All procedures are performed using a range of 4.5 to 16x
magnification. BioPure MTAD was used as the final irrigant. - Kendo
