Subject: [roots] Fractured premolar

Healthy 45 year old female broke her tooth eating popcorn 
2-3 weeks ago. Tooth #29 with a fractured lingual cusp and 
exquisite sensibility to air, touch and brushing.
Ice test :Lingering pain #29 for 2 minutes.....she mentioned 
the tooth had been on and off sensitive in the past, but after 
fracture it kept getting worse....duh! Accessed the pulp, 
filed and flared the canals to a size 35 with a .08 taper file.
No internal or external fractures or craze line noted under the scope
Hand file for apical clearing in presence of catholyte.

Nice, Ahmad. How did the 2nd molar test? I'd sure like to do rct on that one now if it needed rather than down the road when the chamber is even more sclerosed. Which brings up the topic of would it ever be advised to treat a tooth like that based solely on radiographic evidence of progressive canal sclerosis in absence of any negative clinical hx or bad vitality test results? I betcha Terry P could make a case for it! - Mark
