Subject: [roots] Nice Hook

Referral case, pt. complained about discomfort on warm and cold from time to time. Bridge has been placed with
temporary cement for about one year. I could not see any opening into the pulp chamber (could have been happened during
prparation) when I took of the bridge in the first visit. Tooth was sensitive to cold! After replacing the bridge there
were no symptoms for some month, but 2 weeks ago Ihad to do the pulpectomy.

The preop film showed some of the special anatomy: staring in the pulp chamber there is a deeper bifurcation, the the
canals seem to merge, then the canal space disappears at the end. I exspected a very short and severe apical curvature,
so after C&S up to the last 2 mm I used prebended handfiles (fisrt SS then PT handfiles to get around that hook. A lot
of recapitulation to keep WL and patency. Prebended GP-points also. Nice example of how to avoid complications by
having a good look on the preop film.
