Subject: [roots] Huge single canal in lower second premolar
That is a lower right second premolar retreatment.
The RX before is deceiving and one could think it's just another canal.
Took out a single .02 gutta ( may be it was # 30 - a very tiny one )
placed inside, what appeared to be, a huge oval canal. Fortunately
for me it was relatively easy this time to get through the
Russian red paste.
Rest was a lot of shaping with hand files up to #80.02. It was a lot
of rinsing, too. Placed CaOH for one week at the end of first visit.
During second visit (today ) - removal of CaOH, rinsing again with
lot of 2.5 % Hypo, 2 ml 17 % EDTA, again 4 ml Hypo and finished
rinsing with 4 ml 3 % hydrogen peroxide. Filling was done with
bioceramic sealer iRootSP - first used Lentulo filler at 300 turns
to get the sealer inside such a big canal, then used hand spreader
to disperse the bioceramic on canal's walls and finally "condensed"
the ceramic by inserting two gutta percha posts - one # 35.06 and
one # 30.06 - tips reaching one mm short of WL. Final mm of this
# 80 canal is filled with bioceramic only.
Lateral view gives some more information on the extend of
