I could see in the pre-op that the distal canal exists laterally with a curve. Mesial pulp horn had a deep base that was exposed during caries removal, hence causing the demise of the pulp. Not very happy with my apical execution. I would have liked to see more defined and more funneling. No one to blame but me for not recapitulating CZ once more. Basically created a zone with series of larger and shouter length files, but never went back to redefine it. But in my own defense, I could feel the tiny bifidity with scouting file and didn't want to disturb it. At least, that's my excuse! - ahmad
