I zoom in on the pictures sent to me and let my mind absorb everything
that is to be noticed :
- It is difficult to judge the depth of the final clean cavity contour
without a radiograph, probably crown lengthening was not necessary
- Isolation was adequate and apparently served its purpose
- The interdental dam septum was torn by accident or on purpose,
I can not judge. Maybe this could have been avoided by separating
the two teeth a bit more with a fine diamond prior to installing the dam.
If it was by accident it could happen by passing the dam interdentally
with floss, then ripping it with the sharp edge of the distal cavity
border. Maybe it was cut on purpose because the dam could not be held
low enough below the final cavity contour . Things that could help
there without cutting the dam and creating a split dam : teflon tape
and/or retraction chord.
- I guess that the blue stuff is block out resin. It is a wonderful
idea to use it as an aid in achieving final isolation on the critical
margin AND to use it as a matrix for the build-up once the endo is
finished. I think i do not need to stress the advantages of combining
endo and build-up procedure, especially if isolation is tricky or time
consuming. To many times the beautiful work of the endodontist is
screwed up by lousy isolation during the build-up procedure or
even worse : someone taking an impression of the canal to make a post....
