I’ll try to get back on a clinical rather than philosophical track 
which will be less likely to embarrass me. Ken mentioned root anatomy 
and endodontists knowing root sockets.  Here’ is an example of an 
extremely long narrow, curved fractured root.  It would have taken me 
forever elevate/pull this tooth out but the drill out was simpler,
kinder to the bone, and took less than a ˝ hour.  I used a 30mm long 
veterinarian round bur as advocated by Kevin Mullin. Very simple to do 
with a rubber dam.  If it had been a second molar near a buccal 
positioned mandibular nerve where I couldn’t use a dam I would have 
referred it to an oral surgeon.

The second extraction case next to it is a rubber dam extraction of 
a maxillary second molar performed yesterday. The patient is from Maine, 
had a split tooth emergency and will go back to their periodontist in 
Maine for the implant. I communicated with the periodontist to see if 
he wanted me to take the tooth out and prepared it the way he wanted 
with Collacote plugs in each of the three sockets and a sutured Biomend 
membrane. It seems like a good idea for the person placing the implant 
to be the person who extracted it, but in this case the patient
was going to be in town too long,  so I tried to give him as much info 
as possible with multiple pics and a CBCT disk.  - Terry
