Subject: [roots] 3 canal max bi, cracked
Patient complains of of Pain to biting. Referring Dentist wasnt sure, and quite rightly made an immediate referral.
O/E 13 was found cracked. Mesiodistal crack from the occlusal extending into the chamber but not down the root.
RCT completed with a very conservative access to preserve dentin. patient is recommended a cuspal /Full coverage
to prevent the crack from getting worse.
its incredible how many patients i treat have three canals in the maxillar/mandibular bicupids.
As in this patient, you see fairly regularly in patients , all premolars (both upper /lower arches) with three canals.

access, use hand files to 20 to open the orifices,
TF 10, 08 ,06 and 04 files till i can get the 06/25 to length.
hand files in between each rotary to maintain patency.
sequence 04/30 in the buccals to finish and 04/35 in the palatal - Sashi
