Maxillary first premolar retreatment

Blocked canals but with no signs of pathology what so ever. I performed the re treatment. the canals were so difficult to negotiate to full length that i ended up using among other stuff the woodpecker technique as Rafael named it. I was really happy with the result. Almost three years later a lesion is growing at the apex. Somehow i suspected that i created an apical crack there.
