From: John A Khademi DDS MS Sent: Friday, August 26, 2005 4:26 PM To: ROOTS Subject: [roots] Retreat du jour Today: Robin presents with Nec/AAA after having had a root canal and crown done some months ago for what sounded to me like a reversible pulpitis, but I was not there. We gained access through what I thought was a PFM, only to encounter a very, very hard, what seemed like porcelain substructure. If any of the restorative guys know what this is....man it is tough to get through. We located three Thermafil obturators that were poorly placed and were easy to jack out screwing a new 21mm #70 Hedstrom in the palatal, #40 in the DB, and #35 in the MB. Using the plastic mirror handle as a fulcrum and some 45 degree Steiglitz (sp?) (or hemostats) under the handle they often come out in one piece. We located a missed MB2 and MB3. After determining length, an initial shape was created to 25/.06, which in this case was pretty pointless because the canals were so huge. It took so long to get thru the porcelain that I did not have enough time to fully prepare the MB2/Mb3 which join, or use chloroform. Preparation sizes to full EAL length today were: MB=40/.08 MB2=20/.06 MB3=20/.06 DB=40/.08 P=70/.12 At the 1mm short point where Barry continues to accuse me and Steve Buchanan of underpreparing the sizes would be approimately: MB=52/.08 MB2=29/.06 MB3=29/.06 DB=52/.08 P=88/.12 I will re-gauge the MB2/MB3, and probably prepare those to a 25/.06 at full EAL length, which will be about 34/.06 at the 1mm mark. -j
