Subject: [roots] Revascularization?

Iím not making a lot of progress with this kid.  I started this 
revascularization treatment in 2008, placed the triple AB paste, 
stimulated bleeding a month later, placed the MTA barrier, let the
orthodontist jack him around,  I lost control of him/he disappeared 
and had bleaching done (tetracycline component we donítí use anymore), 
then he re-abscessed last year,  I repeated the process (2011) and 
this is what he looks like with the abscess gone today at a recall visit.
His tooth is still discolored.  I plan on going back in, checking 
the status of the pulp, place a little deeper MTA barrier and 
internally bleach with Sodium Perborate.

Big question: Why is the root not forming?

Do we have SHED, SCAP, and DPSCís available for recruitment into 
the pulp space anymore? Has all the interim horseplay eliminated 
the potential for a scaffold and revascularization
process to form an apical elongating root?

Interestingly there is normal mobility (meaning very slight) 
so I donít think the tooth is ankylosed.

Any comments, theories, science, or ideas? How often does a
