RD asked to finish the treatment. 1) Anesthetize: Lower mandibular nerve block with 3/4th of one carpule of Mepivacaine 3%, the other 1/4th was given buccal to the molar. 2) Removal of the temporary restoration and applying the rubberdam. 3) Removal of the distal composite and rest of cariŽs 4) Establishing working length in 3 canals and create a glide path. 5) Check working length with radiograph after using Protaper S1 in all canals. 6) I noted that there must be another distal canal. After searching with prebent files and some Micro-Openers I found it, actually it was a deep apical split of the distal canal. 7) Enlarge canals further with Protapers S2, F1 and Profile 30.06. 8) Final shaping with handfiles 30. 9) Obturation with System B and Obtura (gutta + Topseal) 10) Applying Permaflo Purple composite to avoid contamination and lowering all the cusps in order to oblige the RD to create a restoration with cuspal coverage. 11) Temporary restoration in GIC 12) In the afternoon she had an appointment with her dentist to create a permanent restoration. Cleaning protocol: NaOCL 5%, CA 10%, PUI Patency kept with size 10. Small distal overfilling on post-op was removed afterwards obviously with a simple polishing strip Feel free to comment - Raf Hess anatomy
