Spreader loading

The buccal perio pocket has been watched for years. Tooth originally treated in 1982. Patient said, tooth never really bothered him.much. A Khademi success for 30 years. Just a radiolucency, not a lesion, right? Patient isnít having pain. This is a classic presentation of spreader loading. Lateral condensation is more likely crack roots than the warm vertical technique. Thereís an outside chance this might be a related to lateral/furcation accessories, blowing out the attachment with this type of pattern but highly unlikely. No coronal signs of a crack and it clearly does not appear to be a case of coronal crack propagation. Two changes in technique would likely have prevented this: 1. Better direct line access coronally so that a plugger would not have wedged dentin walls splitting it, yet still allowing convenience form for better fulfillment of endodontic objectives. 2. Better isolation asepsis, irrigation, passive use of files maintaining clearance/patency/better apical cleaning. Terry spreader loading
