I have had three in the last few months. I have looked for additional anatomy, but have been unsuccessful so far. I am attaching slides of one of the cases. Let me know what your thoughts are on the subject? - Joshua C. Gorman, DDSJosh, 2 canal Maxillary 2nd molars are rare...in fact..rarer then 11% in my practice. Typically you would want to take a very distal angled projection (30-45 degrees) and if the patient is cooperative enough, how about a mesial? Look at your original radiograph enlarged...I have drawn in what might be another canal system based on the PDL...I can't tell for sure based on what I got...I'll typically take 4 or 5 post ops changing the angle to see if I see it better... Joey D Joshua, I have had several. If the files go out in the same angle out of the pulpar floor, it's a good sign. I have not come across many with 2 canals, I have had one case with 4 separate roots - 2 palatal canals ( 1.4%). There is also a possibility of finding another canal at times an MB or MB2 canal within the Palatal canal....just getting educated from various sources...(;...the possibilities are endless... - Rajiv Patel Thanks Rajiv.
