I started this t-fill re-tx almost a year ago, and the patient disappeared. Today he returned for completion
and I was shocked at how conservative my initial access prep was.
The first thing I did was enlarge the access (maybe too big?) and boy did this simplify tx. I even found that
on the initial visit last year, I had incompletely troughed the isthmus between the mb & ml. I toughed it out
pretty far down today, and didn't find a mm canal, but I did trough out a bunch of junk that I missed last time.
I tried to get a photo at the distal of the leakage under the core so as to communicate to the referring doc
the need for a new crown. Not a perfect picture, but this was about the 4th try and the patient's adolescent
sons were going stir crazy, about to climb the walls, so I went with this photo - Mark Dreyer
